Instructions to sign up for the Dominican Republic Trip, 2013

Trip Dates

Although the deadline for sign up is April 24, participants will be included on a first-come-first-serve basis. (Limit ~25 each trip). Please contact me if the deadline is a problem and I would love to work with you.  Click on links below for apps and information.

1. Fill out the application and mail to: Decatur Presbyterian Church (DPC), 2306 Modaus Road, SW, Decatur, AL  35603 by April 24.
2. If under 18, also fill out “Consent Form for Minor” form and mail to DPC by April 24.
3. Send check for $750 payable to “Decatur Presbyterian Church”, and earmark with “name” and “DR trip”.  Add $400 for each additional week.  Mail to DPC by April 24. (If you cannot get the money to the church by that date, let’s talk.  I’m very happy to work with you.)  You may be interested in the sample fundraising letter (PDF or Word Doc).  Folks have been super successful raising funds in this way.
4. Get your passport ASAP.  If you already have one, please make sure it has not expired.
5. You must purchase your own plane ticket.  Here is the flight information and I will be happy to walk you through the purchase if you just give me a call.  Insure your spot with me before purchasing.  Purchase ticket from Delta.com or other travel site but must be the Delta flight.   Please email me your flight itinerary once purchased.
6. Read “What you need for the DR” so you’ll know what to pack.  Especially note the shots required, the dress code, and the need for water bottles and bug spray.
7. Many insurance policies cover overseas medical needs, including med-flight.  Check to see if yours does.  If not, individual short-term insurance must be purchased for the week for a very modest price (~$20).  Please send me a copy of your insurance card if you go this route.

P.S.S.  Our specific building project this year will be the new school at the land  we purchased in El Cacique, the village at the dump.  Plus there will be Bible schools, Bible studies, shopping, medical brigades at the bateys and tons more.
